The Australian Transport Safety Bureau (ATSB) has released a final report into an incident where a Qantas Airbus A380 flew two international sectors with a maintenance work light left inside its left wing. The report states that the light was inadvertently left in the wing following maintenance on the aircraft's air conditioning system.
## Incident Overview The incident occurred on January 7, 2026, when the A380, registered VH-OQK, was undergoing scheduled maintenance at Sydney Airport. The maintenance work involved the aircraft's air conditioning system, and it was during this time that the work light was left inside the wing. The aircraft then flew to Dallas/Fort Worth International Airport and back to Sydney before the light was removed on January 9, 2026.
The ATSB report notes that the aircraft sustained no damage and no injuries were reported as a result of the incident. The report also highlights the breakdown in foreign object control and tool accountability processes that led to the incident.
## Investigation Findings The ATSB investigation found that there were three separate opportunities to catch the missing light, but it was not flagged on any of these occasions. The maintainer did not find the light during the foreign object clearance inspections at the end of the work, and the light's absence was not noticed when the maintainer returned tools or during the unreturned tool check at the end of the shift. A licensed aircraft maintenance engineer then issued a certificate of release to service while the work light remained unaccounted for.
The report attributes the incident to a breakdown in foreign object control and tool accountability processes, according to ATSB Chief Commissioner Angus Mitchell. The finding is the second time in two years that the ATSB has reported on a tool control issue involving Qantas.
